Records show that a Westerville police officer who missed a gun during a pat-down of Miles Jackson had been previously encouraged to not rush his job responsibilities.
Jackson died Monday after a struggle with officers over the weapon in an emergency room exam room at St. Ann's Hospital.
Body camera footage shows Westerville Police Officer Eric Everhart conducting a pat-down on Jackson and asking if he has any weapons.
Everhart's personnel files include references to supervisors encouraging him to slow down and think things through.
Everhart and another Westerville police officer have been placed on administrative leave following the shooting.
A police union official commended Everhart's actions as professional and compassionate.
Body camera footage released on Wednesday shows a more than a six-minute struggle with Jackson within an emergency room exam room that led to police officers shooting him multiple times. Right before police fired multiple shots at Jackson, a single shot was fired.
The video initially shows two Columbus police officers struggling to subdue and retrieve a gun that Jackson had in his possession. One police officer used a taser on Jackson when his gun fired an initial shot.
As police searched Jackson, they found a gun and a bullet in his pants.
The names of the Columbus Police officers who fired shots at the 27-year old Jackson have been released. Columbus Police identify them as Andrew Howe and Ryan Krichbaum, both are 15-year veterans of CPD.
Security officers involved from Mount Carmel St. Ann's hospital have not been identified.
